This isn't hp hacking. He instantly lost hp as he got knocked by shotgun and he didn't receive high damage from grenade because it was too far from the position he was standing and he definitely missed your shotgun hit. It happens, sometimes shots like this don't sync, it is normal and can occur quite frequently.
